People with dementia (PwD) along with their caregivers currently face challenges in the disease process because there is a major gap in available treatment and therapies, especially those that are non-pharmacological. The literature shows an increased incidence of dementia, and currently, there is no cure. This leaves medication as the treatment of choice. The aim of this project was to investigate whether introducing robotic pets as a supplemental therapy to persons with mild to moderate dementia helps in reducing their level of anxiety and agitation. Based on a review of the literature on the benefits of robotic pet interactions with dementia-suffering individuals, a quality improvement project was conducted in a suburban city in Arizona. Project participants included individuals with dementia and their caregivers. They were asked to interact with the robotic pet in their home for four, one-hour sessions, over a four-week timeframe. Analysis of the findings revealed that individuals with dementia found a sense of joy and peace while interacting with the pets, and they looked forward to the pet visit every week with excitement. These results indicated that introducing robotic pets to dementia-suffering individuals was clinically significant as it relates to their anxiety and agitation levels. Utilizing robotic pets as a supplemental therapy can benefit dementia-suffering individuals and their caregivers. Further evaluation is needed with a larger sample size to better understand the impact of robotic pets on PwD.

Purpose & Background: This project evaluates the impact of education on a provider’s intent to screen for vitamin D deficiency in adults with depression. An internal Quality Improvement (QI) study at a local mental health primary care clinic revealed that only 1 in 3 patients with depression were routinely screened for vitamin D deficiency. Vitamin D is a crucial component of numerous systemic functions, including mental health, specifically depression. Methods: This QI project used the Rosswurm and Larrabee Model implementation framework. Institution Review Board (IRB) expedited review approval was received. This project was conducted at 10 Veteran’s Affairs (VA) primary care clinics in Arizona. An initial email with a recruitment flyer was sent to providers to launch the project. A second email was sent to participants who volunteered to participate in the project, with instructions and links to the asynchronous pre-survey, recorded education PowerPoint, and post-survey. Responses were analyzed using Intellectus Statistics software. Results: Provider knowledge of impact and intent to screen for vitamin D deficiency increased after viewing a brief education video (n=30). Frequency distribution analyses revealed a 23% average increase in agreement to screen for vitamin D deficiency at annual visits, “at-risk” individuals, knowledge of the association, and intent to screen regularly in depression. Conclusion: The education intervention was found to positively impact the provider’s intent and demonstrate the importance of screening for vitamin D deficiency in adults with depression.

Aim: To evaluate the impact transformational leadership (TFL) behaviors and What Matters to You conversations have on RNs finding meaning and joy in work (MJW) and turnover. Background: The nursing profession is plagued by burnout - a precursor to loss of MJW. Loss of MJW was exhibited as low morale and increased turnover among acute care RNs at a small hospital in Southwest Arizona. Addressing loss of MJW aligns with caring for the caregiver, the fourth aim of the quadruple aim initiative. Methods: This was a quasi?experimental mixed methodology evidence-based project. The target populations were core RNs and leaders working in the intensive care unit, care unit, and emergency department. Intervention was multimodal – survey using Meaning and Joy in Work Questionnaire, TFL education, and steps one and two of the IHI four steps for leaders model. Results: Final sample was 18 RNs. Statistical analyses did not reveal significant impact; pre- and post-survey MJWQ scores remained above four. Themes from the What Matters to You conversations included making a difference, coworkers/connections, staffing, and negativity. Turnover trended positively in two of the three units. Conclusion: This project heightened awareness about MJW and illuminated the impact TFL behaviors can have on RNs finding MJW and turnover. The coronavirus pandemic and acute nursing shortage were significant limitations of the project. Implications: Healthcare organizations are encouraged to view MJW as a system asset, embed it in their cultures, invest in innovative solutions, and continually evaluate outcome measures of MJW.

While the COVID-19 pandemic continues to evolve, America’s nursing work force continue to work in the most challenging of circumstances. While expected to hold the fort and continue on, deep inside, they bury an unprecedented level of acute stress, anxiety and depression. Peer support groups have been posed as a possible coping behavior. This cross-sectional designed project was developed to assess the worth and feasibility of a virtual peer support group with a focus on healthcare provider wellness during a period of surge of the COVID-19 pandemic. Overwhelmed staff, technology/documentation changes and challenges, competing clinical demands, short-staffing and Zoom fatigue were identified as the limiting factors for this project’s completion within its given timeframe. These findings informed of current barriers, providing a basis for future program development to mitigate the impact of psychological distress among healthcare providers. Evolving literature on this topic supports recommendations for further study and action by individual health care providers, organizations and at the state and national levels.

Healthcare workers may have poor attitudes, bias or feel unprepared when caring for patients with mental illness in the pediatric intensive care unit (PICU) setting. The aim of this project was to resolve these biases using education to increase quality of care for pediatric mental health patients using Rossworm and Larabee’s change model. The Questionnaire on Stigmatizing Attitudes Towards Children with Emotional and Behavioral Disorders (EBD) was completed by voluntary participants at pretest, posttest and at a one month follow up. Between pretest and posttest, participants engaged in three educational modules regarding mental health. Participants were voluntary, anonymous staff members at a pediatric hospital currently employed in the PICU. Results of the questionnaires pre M= 23 post M=11 and follow up M=9.5 indicates education improves attitudes, and decreases bias in PICU staff. Recommendation to obtain larger population of study with trial implementation. Recommend adding educational modules to annual education or to new hire orientation.

Burnout has become an increasingly popular topic among registered nurses, but unfortunately burnout among psychiatric nursing is less understood than other nursing specialties such as the Intensive Care Unit, Emergency Room, or Oncology. Psychiatry is unique and psychiatric nurses, in particular, are often subjected to physical and verbal violence as well as exposure to patient’s trauma. The aim of this project was to decrease burnout among psychiatric nurses in a private practice out-patient family psychiatric facility using Rossworm and Larabee’s change model (Appendix D). The MBI-HSS was completed by 1 participant (n=1) at pre-intervention and post-intervention. Between the pre/post MBI-HSS questionnaire the participant was asked to partake in a mindfulness-based intervention utilizing the smartphone application Headspace to complete a 10-session meditation course over one week. The results conclude the participant’s burnout decreased overall from pre-intervention to post-intervention. Internal Review Board (IRB) was granted in September 2021, and the project was completed in November 2021. The impact of the project was projected to have a more thorough statistical influence, but due to the participant size, there is minimal impact of system or polices in the psychiatric facility.

Purpose: The COVID-19 pandemic has increased depression among the population. Exercise is a useful coping mechanism for depression and high intensity interval training (HIIT) can be utilized. Background: A community exercise facility provides HIIT exercises for gym members. The gym owner believed their HIIT program is a great tool for coping with depression. Methods: The gym offered a 21-day free HIIT program. Those who signed up for the program, 18 years of age or older, and were able to exercise under the Physical Activity Readiness Questionnaire were offered to participate in the study. Participants were given a pre-survey before they started exercises. This survey utilized the Patient Health Questionnaire-9 (PHQ-9), a standard depression rating tool that determines depression symptom prevalence. After the 21-day program, participants were given a post-survey involving the same PHQ-9. The survey scores were compared pre- to post-program. The surveys were coded to exclude any identifying information. Results: Mean PHQ-9 pre-test was 7.67, mean score post-test was 3. A paired t-Test resulted in a p=0.60, showed that it was not statistically significant. Discussion: The goal of this study was to determine if HIIT training was an effective coping mechanism for depression. The data shows that participants had decreased PHQ-9 scores from the pre- and post-program surveys showing a clinical significance. This data can be used to provide those suffering from depression a coping mechanism.

Purpose: The COVID-19 pandemic government regulations and mandates have caused feelings of overload, isolation, anxiety, depression, and loneliness. This project aimed to evaluate the level of impact of social distancing and homestay mandates on seniors during the COVID-19 pandemic. The knowledge from the data guided the implementation of Information Communication Technology (ICT) as a focused intervention to address loneliness, social isolation, and depression among seniors. Methods: Quantitative reviews were employed to assess the impact of ICT on lonely seniors. A convenience sample of 5 seniors aged 65 and up from a local church congregating in worship center located in southwestern United States enrolled in this voluntary pre-and post-educational intervention project. Participants were informed of the confidentiality of the study and that the study had no known risks on participants. The participants willingly signed a consent for the study. The attendees received two one-hour education sessions on how to use WhatsApp and Zoom as communication strategies. The pre-and post-loneliness scale scores were collected using the Revised UCLA Loneliness Scale as a tool. Results were compared before and after the educational intervention using a paired Wilcoxon signed-rank test. Results: The results indicate significant reductions in reported loneliness from pre to post intervention (z = -2.02, p = 0.043). These results indicate that using ICT can help address loneliness in the seniors. Conclusion: Implementing communication strategies such as WhatsApp and Zoom effectively addresses social isolation and loneliness in seniors. The data reveals that integrating electronic communication in the life of the seniors can manage the social isolation problem. Future nursing practices may benefit from the project's data to address loneliness in the seniors.

Background: Advance Care Planning (ACP) conversations are discussions between patients, providers, and loved ones addressing key care decisions in the event of incapacity. Nearly twothirds of US adults have not completed an Advance Directive (AD), yet ACP conversations rarely occur in practice. The objective of this quality improvement project was to implement workflow changes with a reminder system to facilitate ACP conversations during Medicare Wellness Visits (MWV). Method: Social Cognitive Theory describes the complex relationship between variables that can influence an individual’s decision to address ACP. Providers in a primary care office in the Southwestern United States participated in an ACP education session and confidence survey. Patients presenting for the MWV were screened for ACP, and visual reminders were attached outside the exam room for provider review. Aggregate data were used to evaluate provider surveys. Descriptive statistics were used to evaluate patient characteristics and the Chi-square Test of Independence, and Fisher’s test was used to compare the pre-and post-intervention advance directive documentation. Results: Qualitative feedback from the survey indicates reminders and easily accessible resources may help facilitate ACP conversations. Of the 251 MWVs, 21 (8%) had an AD documented, significantly less than the nationally reported rate of 37.7% (p < 0.05, z = -2.39). Conclusions: Healthcare providers face multiple barriers preventing or delaying ACP conversations in practice. System-level changes and provider education can improve the rate of ACP conversations and impact patients’ care at the end of life.

Background: Breastmilk is the optimal source of nutrition for infants as recommended by the American Academy of Pediatrics and the World Health Organization (WHO). Despite proven benefits, rates of breastfeeding have shown to decrease drastically at six months of age. Purpose: The purpose of this project is to assess the effectiveness of virtual peer support and infant education in increasing/improving a mother’s wellbeing and breastfeeding relationship. This project was guided by the theory of self-efficacy. Methods: Participants were recruited utilizing flyers that were posted to social media and distributed at a breastfeeding center in Southwestern United States. Six breastfeeding mothers were recruited, and each participated in four, one-hour peer support sessions that were held weekly via Zoom. Participants completed the WHO Well Being Index and the Breastfeeding Relationship scale before and after the intervention. Pre- and post-implementation scores were compared. Ethical Considerations: The research was approved by the IRB board at Arizona State University and was in compliance with ethical considerations and standards of the institution. Results: Descriptive analysis conducted on the WHO Well-Being Index showed the same results or improvement for 80% of the questionnaire. Descriptive analysis conducted on the Breastfeeding Relationship Scale showed improvement on 81% of questions. Conclusions/Discussion: Breastfeeding can be taxing and overwhelming for mothers leading them to discontinue earlier than planned. Peer support has been identified to be substantial to women going through this process in helping them initiate and continue to breastfeed their infant.
